The new points are PTC's typical endpoints on all circles.  We've had myriad discussion on the purpose for these, but suffice it t say that in Creo, these are valid vertices for connectivity.Alvin Haha...

The *.avg file is not a user-readable file, which lists the h-nodes and the corresponding p-elements where stress averaging should not occur due to shell tangent discontinuity.

Alvin, If you want the mesh to look like your desired post, you will have to rotate your hole 90-degrees.  I've posted the solution below, as I think this is the type of mesh you're looking for.

Hi all,PTC has finally acknowledged this is a fault in the software. It is registered under SPR 2258467 in the knowledgebase.Hopefully it will be fixed in the next datecode! Regards,Agnes
